一、計劃內新生嬰兒入戶Registration of Planned Newborn Babies
前置條件：計劃內新生嬰兒入戶。（只適用于在中國大陸地區出生的新生嬰兒，不適用于在中國香港、澳門、臺灣地區出生的新生嬰兒）
Precondition：Registration of planned newborn babies (Applicable to babies born in Mainland China only; not applicable to babies born in Hong Kong, Macao, and Taiwan China)
1、《入戶申請表》（公安辦證中心現場領取或網上下載）
Household Registration Application Form (obtained from Public Security Bureau Accreditation Center or downloaded online)
2、《出生醫學證明》（原件和復印件）
Medical Birth Certificate (original and copy)
3、《成都市符合法律法規生育通知單》（出生后應先到街道辦事處計劃生育辦公室作出生統計后，再申報戶口）（原件和復印件）
Chengdu Legal Birth Notice (After birth, household registration shall be applied after being included in the population data of the Family Planning Office of Street Office.)(original and copy)
4、父母雙方的結婚證、居民戶口簿、居民身份證（原件和復印件）
Marriage certificates of father and mother, household registration booklet, resident ID cards (original and copy)
二、國外出生嬰兒入戶 Registration of Babies Born Abroad
前置條件：父母其中一方系本市戶籍居民，子女在國外出生，且未取得外國國籍和永久居留權，回國入戶的。
Precondition: One of the parents is a resident of Chengdu. The baby is born abroad but does not obtain foreign citizenship and right of permanent residence; thus, returning to China and apply household registration.
1、《入戶申請表》(公安辦證中心現場取領或網上下載)
Household Registration Application Form (obtained from Public Security Bureau Accreditation Center or downloaded online)
2、新生嬰兒的中國護照或旅行證（原件和復印件）
Chinese passports or travel permit of newborn babies (original and copy )
3、新生嬰兒在國外的出生證和翻譯件（出生后應先到街道辦事處計劃生育辦公室作出生統計后，再申報戶口）（原件和復印件）

6、父母雙方的結婚證（國外領取結婚證的，應在婚姻締結地辦理公證和該國外交部或外交部授權的機關認證，并經中國駐該國使、領館認證）（原件和復印件）
Marriage certificates of father and mother (For those who get marriage certificates abroad, the marriage certificates shall be notarized by the agency of the place where they got married and be certified by the Ministry of Foreign Affairs or the agencies authorized by the Ministry of Foreign Affairs. The marriage certificate also should be certified by Chinese ambassador to the foreign nation and Chinese consulate to this nation.) (original and copy)
三、計劃外出生嬰兒入戶Registration of Unplanned Newborns
前置條件：計劃外出生的子女（含已婚無計劃生子女，非婚生子女）入戶
Precondition: Registration of unplanned newborns (including unplanned children of legal marriages and illegitimate children).
所需材料：
1、《入戶申請表》（公安辦證中心現場領取或網上下載）
Household Registration Application Form (obtained from Public Security Bureau Accreditation Center or downloaded online)
2、《出生醫學證明》（無《出生醫學證明》的提供縣級以上公證機關親子關系證明）（原件和復印件）
Medical Birth Certificate (For those without Medical Birth Certificate , parent-child relationship proof shall be provided by notary organs above county level.) (original and copy)
3、父或母的居民戶口簿、居民身份證（非婚生子女入戶提供）（原件和復印件）
Marriage certificates of father or mother, household registration booklet, resident ID cards (used to register illegitimate children) (original and copy)
4、父母雙方的結婚證、居居戶口簿、居民身份證（已婚無計劃生子入戶提供）（原件和復印件）
Marriage certificates of father and mother, household registration booklet, resident ID cards (used to register unplanned children of legal marriages) (original and copy)
5、征收社會撫養費決定書和繳納社會撫養費的憑據（原件和復印件）
Written decision of collecting social support fee and proof of paying it. (original and copy)
